Total Health Care LLC (“THC”) websites, applications (including mobile applications), and other offerings from THC, (“Platform”) operated by THC LLC (“THC”) enables health facilities operators to run their day-to-day operations, and healthcare service providers (“Healthcare Providers”) provide medical related services (“Healthcare Services”) to patients (“Healthcare Seekers) and for Healthcare Seekers to access Healthcare Services.
Our Privacy Policy is intended to describe to you how and what data we collect, and how and why we use your personal data. It also describes options we provide for you to access, update or otherwise take control of your personal data that we process. Throughout this Privacy Policy we may refer to THC as “us” or “we,” and to you as the “user” or “customer.”

INFORMATION WE COLLECT
THC cares about your privacy. For this reason, we collect and use personal data only as it might be needed for us to deliver to you our world-class products, services, and Platform (collectively, our “Services”).
Our Platform typically collects three kinds of information about you: (a) information that you provide that personally identifies you; (b) account related information; and (c) information that does not personally identify you, but that we automatically collect when you visit our Platform or that you provide us.
- Personal Information: We may receive and store any information you enter on our Platform or provide to us in any way. For instance, depending on your category of use (i.e. either as a Healthcare Provider or Healthcare Seeker) you will be required to provide, but not limited to the following: (a) your name, email, address, phone number, allergies, medical reports, medical histories, blood group, genotype, next-of-kin, license status, other data that could directly or indirectly identify you etc. when registering with our Platform or creating a profile; or (b) provide certain demographic information about you (e.g., age, gender, education, etc.). This information is required to enable us deliver on the Services on our Platform.
- Account-related information is collected in association with your use of our Services, such as account number, purchases, when products renew or expire, information requests, and customer service requests and notes or details explaining what you asked for and how we responded. Cookies and similar technologies on our Platforms and our mobile applications allow us to track your browsing behavior, links clicked, items purchased, your device type, and to collect various data, including analytics, about how you use and interact with the Platform and our Services. This allows us to provide you with more relevant product offerings, a better experience on our Platform, and to collect, analyze and improve the performance of our Services. We may also collect your location (IP address) so that we can personalize our Services. Data about Usage of Services is automatically collected when you use and interact with our Services, including metadata, log files, cookie/device IDs, and location information. This information includes specific data about your interactions with the features, content, and links (including those of third parties, such as social media plugins) contained within the Services, Internet Protocol (IP) address, browser type and settings, the date and time the Services were used, information about browser configuration and plugins, language preferences and cookie data, information about devices accessing the Services, including the type of device, what operating system is used, device settings, application IDs, unique device identifiers and error data, and some of this data collected might be capable of and be used to approximate your location.

Non-personal information can include certain Personal Information that has been de-identified; that is, information that has been rendered un-anonymous. We obtain non-personal information about you from information that you provide us, either separately or together with your Personal Information. We also automatically collect certain non-personal information from you when you access our Platform through our cookies and other similar technologies. This information can include, among other things, IP addresses, the type of browser you are using (e.g., Internet Explorer, Firefox, Safari, etc.), the third party platform from which your visit originated, the operating system you are using (e.g., Vista, Windows XP, Mac OS, etc.), the domain name of your Internet service provider (e.g., CenturyLink, Google, Cox, etc.), the search terms you use on our Platform, the specific web pages you visit, and the duration of your visits. We may also use this information to later contact you for a variety of reasons, such as customer service, providing you promotional information for our products or those of our parent company, subsidiaries or other affiliated companies (“Affiliated Companies”), or to communicate with you about content or other information you have posted or shared with us via our Platform. You may opt-out from receiving future promotional information from us or our Affiliated Companies, or direct that we do not share your information with any Affiliated Companies, as set forth below.
In certain instances, we may also share your Personal Information with other users for the purpose of providing relevant services to you or our third party vendors performing functions on our behalf or other Users (or on behalf of our Affiliated Companies) – e.g., vendors that process credit card orders, deliver our merchandise as it relates to our Services, administer our promotions, provide us marketing or promotional assistance, analyze our data, assist us with customer service, etc.. In addition, we may share your Personal Information with participating sponsors to a program or promotion you enter via our Platform. Further, we may share your Personal Information with third parties such as our co-promotional partners and others with whom we have marketing or other relationships. Platform analytics. We use multiple web analytics tools provided by service partners to collect information about how you interact with our Platform, including what pages you visit, what site you visited before visiting our Platform, how much time you spend on each page, what operating system and web browser you use and network and IP information. We use the information provided by these tools to improve our Services. These tools place persistent cookies in your browser to identify you as a unique user the next time you visit our Platform. Each cookie cannot be used by anyone other than the service provider (ex: Google for Google Analytics). IP Addresses: An IP address is a number that is automatically assigned to your computer whenever you are surfing the Internet. Web servers (computers that “serve up” web pages) automatically identify your computer by its IP address. When visitors request pages from our Platform, our servers typically log their IP addresses. We collect IP addresses for purposes of system administration, to report non-personal aggregate information to others, and to track the use of our Platform. We may offer chat rooms, blogs, message boards, or similar public and private forums where you and other users of our Platform can share information and communicate – e.g., communicate with doctors, pharmacists and other medical professionals. The protections described in this Privacy Policy do not apply when you provide information (including personal information) in connection with your use of these public or private forums. We may however use personally identifiable and non-personal information about you to identify you with a posting in a public or private forum. Any information you share in a public forum is public information and may be seen or collected by anyone, including third parties that do not adhere to our Privacy Policy. All private communications with medical professionals on and/or off the Platform are protected by doctor/patient privilege and as such we will not be liable for the disclosure of such information to third parties.

We have implemented security measures we consider reasonable and appropriate to protect against the loss, misuse and alteration of the information under our control. Please be advised, however, that while we strive to protect your Personal Information and privacy, we cannot guarantee or warrant the security of any information you disclose or transmit to us online and are not responsible for the theft, destruction, or inadvertent disclosure of your Personal Information.
